Department Overview:

The Care Management (CM) department is pivotal in providing proactive and coordinated care to seniors with chronic conditions. As healthcare shifts towards a more patient-centered approach, our CCM team is at the forefront of enhancing the quality of life for these individuals, managing the complexities of chronic diseases with efficiency and empathy.

Key Responsibilities:

Leadership and Communication:

  • Directly manage and oversee a growing team of 50-60 care managers within the CM

department, with potential for team expansion as our operations scale.

  • Serve as the primary contact for Medical Directors, CMOs, and client physician groups, ensuring

a seamless flow of communication and collaboration.

  • Conduct strategic meetings to maintain and enhance the quality of care management services

provided.

Operational Management:

  • Oversee the recruitment and training of new Medical Assistants and other key personnel,

focusing on building a team that aligns with our values of patient-centered care.

  • Manage daily operations within the CM department, ensuring all activities align with our goals

of delivering better health and care at reduced costs.

  • Expertly navigate and utilize care management systems, with a primary focus on ThoroughCare,

to enhance service delivery.

Strategic Initiatives:

  • Collaborate with cross-functional teams, including clinical pharmacists and community health

workers, to implement comprehensive care solutions.

  • Adapt and innovate care management strategies to meet the evolving needs of our patient

population and the broader healthcare landscape.

Qualifications:

  • At least 10-15 years of experience in healthcare management, particularly within an ACO, care

management, or chronic care management setting.

  • Proven track record of managing large, dynamic teams and complex operations.
  • Strong public speaking skills and the ability to effectively communicate with high-level stakeholders.
  • Extensive experience in using care management systems to improve healthcare delivery.
